Admissions > Application Forms The UW-L – Gundersen – Mayo PA Program is a program that participates in the Centralized Application Services for Physician Assistants (CASPA), and requires a program specific Supplemental Application. The CASPA Application is easily completed online using their web-based data entry system. The UW-L – Gundersen – Mayo PA Program Supplemental Application form is available for download using the links below. The Instructions, Supplemental Application and Application Checklist are downloadable as Acrobat Reader files (*.pdf). The files are optimized for use with Acrobat Reader 6.0 or higher which is available here. The Supplemental Application is easily completed using Acrobat Reader, but then must be printed and signed. It is then the paper copy of your application that is submitted to the program. The following links provide access to the forms that are used for the 2008 process. Materials for the class matriculating in June 2008 can be sent after June 1, 2007.
